What We Test Before Prescribing TRT

Men’s testosterone declines gradually from the mid-30s and usually shows itself by the early 40s. Women lose testosterone too, most sharply around menopause. And it rarely drops alone: thyroid, stress hormones, and blood sugar all shape how you feel.

A standard panel often stops at total testosterone. Ours doesn’t.

Marker Why we test it
Total and free testosterone Total shows your overall level; free shows the portion your body can readily use.
DHT A more potent hormone made from testosterone. It influences libido and other traits, so we monitor it.
SHBG and estradiol SHBG affects how much testosterone stays available. Estradiol matters in men too, and TRT can raise it, so we track the balance.
PSA (free and total) A prostate safety marker we check before and during treatment.
Cortisol and DHEA-S Stress and adrenal hormones that interact closely with testosterone and energy.
Thyroid (Free T3, Free T4, TSH) Thyroid issues can mimic low testosterone, so we rule them out first.
Metabolic markers (fasting insulin, fasting glucose, HbA1c) Testosterone and blood sugar regulation often move together, so we assess them alongside.

Because low testosterone affects bone and heart health, we can add a DEXA bone-density scan, a coronary calcium score, or cardiometabolic markers.

If TRT is right for you, your doctor sets the dose and delivery method: pellets that last 3 to 6 months, injections, creams, gels, or patches. We recheck levels every 3 to 6 months, then twice yearly.

If your labs don’t point to testosterone, we tell you, and we keep looking for what does.

How Long Should You Be on TRT?
It depends on your lab results, your response to treatment, and your long-term health goals. Your provider reviews your plan at follow-up visits every 3-6 months. They make adjustments as needed.

Who Should Avoid TRT?
TRT is not safe for everyone. It may not suit people with certain health conditions or abnormal lab findings. Our Los Gatos clinic requires a careful medical history and a full lab review. This helps us confirm whether TRT is appropriate and safe for you.

When Will I Start Noticing the Effects of TRT?
Response varies from person to person and depends on baseline health, treatment plan, and ongoing monitoring. Your provider will discuss expectations based on your history and monitor progress over time.
